The Advanced Care Clinic is open 24/7 to provide expert care for patients who need assistance after being discharged.
If you start to feel unwell after leaving the hospital, please refer to the instructions in your discharge paperwork or after-visit summary in MyChart.
Depending on your symptoms, your doctor may refer you for an appointment at the Advanced Care Clinic.
Our clinic offers round-the-clock diagnostics and specialty care for patients referred after a treatment, procedure or surgery.
Patients we serve
The Advanced Care Clinic is here for:
Patients currently under the care of a UT Health San Antonio surgeon or specialist who has referred them for an appointment
Patients with cancer who have been referred by their UT Health San Antonio oncologist
We prioritize your safety and well-being and do not provide appointments for life-threatening emergencies. Please call 911 or visit your nearest emergency room if you are experiencing
Cardiac or heart concerns
Stroke symptoms
Traumatic injuries
Acute psychiatric issues

A physician's referral is required. Please request a referral from your UT Health San Antonio specialist or surgeon.

What to expect
Before your visit: Please reach out to your UT Health San Antonio provider to share your symptoms and needs. They may refer you to the Advanced Care Clinic.
During your visit: Our compassionate team of experienced professionals will evaluate your condition, perform any necessary lab tests or imaging and collaborate with you to create a personalized care plan.
After your visit: You will receive clear instructions and follow-up recommendations. If further care is needed, we will ensure a smooth admission process.
