Create a MyChart login
New to MyChart? There are two ways to create your account online:
- Sign up with your MyChart activation code. Find your activation code on your enrollment letter or After Visit Summary. You may call your provider’s office to request an activation code.
- Sign up online without a code. You can create a MyChart account, even without an activation code. You will just need to answer a few more questions.

What is MyChart?
UT Health San Antonio offers MyChart, an online health portal that securely stores your electronic medical records. Available from your smartphone, computer or tablet, this online system makes it easy for your care teams to update and collaborate on your medical information.
At UT Health San Antonio Multispecialty and Research Hospital, we use the same MyChart system as your providers at all our other UT Health San Antonio care sites, streamlining their collaboration and enhancing your overall care.
How can I use MyChart?
Use MyChart to complete these tasks:
- View your medical record: See lab results, prescriptions, images, appointments, and more.
- View your care team: Keep a record of the providers you have seen at UT Health San Antonio.
- Pay a bill: See your bills and pay them online. Learn more about bill pay.
- Request a refill: Need more medication? Send a request with your MyChart account.
- Schedule an appointment: Set up a new appointment or view details about current appointments.
- Send a message to your care team: Ask questions or send replies using a simple chat interface.
- View test results: Get results fast from lab tests and imaging and radiology scans.
